Maybe I'm wrong, but I seem to recall this as being one of the first Death Guard models that established the look and feel of the chapter. I think it was sculpted by Jes Goodwin. This seemed like a good model to start my gallery with, it was also the model I chose to use in my Plague Marine painting advice article though this is not the exact paint job.

This paint job was done while I was living with Owen in Victoria. It is part of Jaundice squad though it is too green. Jaundice should be more grey, but I did one too many washes and probably too many unthinned washes... The key color was Ghoul Grey which I got from the Creature or Monster paint set. It is pretty much dried out so painting more members of this squad is difficult. I may paint up another 3-4 using a lot of Rotting Flesh to achieve a paler effect then my usual green Plague Marines. What I'm going for is seen on the sword of my OOP Terminator Lord which I hope to experiment with on Plaguebearers and will probably use on my Forgeworld Dreadnought. This was done by using Camo Green and mixing in more and more Rotting Flesh. I also used some thinned down washes.
